I, _______________________________, acknowledge responsibility for the Celebration Ministry Trailer between and including the dates listed above as pick up and return dates. I have read and am in agreement with the CMT (Celebration Ministry Trailer) Policies and will return the CMT at the above agreed upon time or will call if there is a delay. I agree to the policies concerning reporting and repairing/replacing any damaged or lost equipment on or connected to the trailer. I understand that the trailer is the sole responsibility of our church between the dates inclusive when we have reserved the trailer.

As representative of the borrowing church, I voluntarily release, indemnify, hold harmless and blameless the Sandhills Baptist Association, its other member churches, and its employees from all liability, claims, demands, actions, or rights of actions, whether personal to me or to a third party which are related to, arise out of, or are in any way connected to our use of the Celebration Ministry Trailer and its components, including those allegedly attributable to negligent acts or omissions. I agree to reimburse any reasonable attorney fees and costs which may be incurred by the Sandhills Baptist Association from whom I am borrowing the trailer in the defense of any such liability claim, demand, action, or right of action.
